Dr. Jenna Schulten grew up in Campbellsville, Kentucky and graduated with a Bachelor of Biology from the University of Louisville. She graduated from Centre College with a BS in behavioural neuroscience and Spanish, and then received an M.B.A. in psychology from Kentucky State University.
After graduating, she decided to pursue dentistry and continued her academic efforts at the University of Kentucky, where she earned a certificate in pediatric dentistry. After a year and a half as a dental assistant, she decided to continue her education and earned her doctorate after completing her dental school. She continued her efforts with advanced academics, earning an M.B.A. in Psychology from Kentucky State University and then a B.S. with a minor in Psychology from the College of Humanities and Social Sciences at Kentucky University, before graduating with a D, M and D in Pediatrics from Louisville Children's Hospital in Louisville, Kentucky. After graduating from dental school, she completed a Master of Science in pediatrics and pediatric dentistry at the University of L's School of Pediatrics. She also completed two years of postdoctoral studies in pediatric dentistry at the University of Louisville Medical Center, where she earned her DMD in pediatrics and her Master of Pediatrics.
Dr. Kathy J. Pansegrau also works with a medical group that includes her Dc. She attended and graduated from Cleveland Chiropractic College in Kansas City with a bachelor's degree in psychology and a master's degree in social work. She is also a member of the Board of Directors of the University of Louisville Medical Center and the Kentucky State University College of Medicine.
When she's not practicing dentistry, Dr. Haeberle enjoys spending time with her husband, Tye, and enjoying outdoor activities, including long-distance running. In her free time she likes to hike, camp and play with her three dogs JR, Jasey and Earl. Outside the office, Dr. Jenna enjoys traveling, spending time with friends and family, trying out new restaurants, enjoying the outdoors and cheering on the Louisville Cardinals.
If the Company is held liable in any way under this Agreement, the Releasor agrees that the total amount payable to the Company in the previous twelve (12) months shall be equal to or less than the damage suffered by the Releasor. Likewise, participation in any of the activities may enable any other participant or third party to communicate or send a message. Should any of these activities cause harm to Relesor or to any person or entity or in connection with any matter for which it is liable, it acknowledges that the Company shall not be liable for any damage it may incur, directly or indirectly, as a result of such activities.
This paragraph should not be construed as an attempt to waive or release rights or claims which cannot be in any way exempted from their right or entitlement under the law. If this Agreement is not amended, the infringing provisions will be removed and the remainder of the Agreement will remain in its current form and will be interpreted to the best of our knowledge and belief to achieve its intent as set forth in this Agreement.
The company does not expect disputes with the releasers, but in cases that occur, these issues are addressed and the solution often takes the form of punitive or compensation payments. If there is any other aspect of your relationship with Re Leasor Company that leads to you taking legal or quasi-legal action, you will apply Kentucky law and regulations to the extent permitted. Civil litigation is a private dispute between persons or organizations and may only be brought before the courts of Jefferson County, Kentucky.
The parties confirm that this Agreement is final and complete and that no other information contrary to the Agreement shall replace it. This Agreement may not be amended without the prior written consent of Re Leasor Company unless you clearly intend to amend it or the party confirms that it is both final and complete and will be replaced by another agreement or other legal action. The agreement to use electronic signatures for all documents shall remain valid until you notify us in writing that you no longer wish to use electronic signatures. Please make sure you have your current e-mail address to contact us if necessary about any changes.
Re Leasor will not file any lawsuit or other complaint against you for matters resolved or disclosed herein or against any other person or entity.
Re Leasor undertakes not to participate in any activity of your Services, either personally or online, without your consent or consent, that the Releasors are fully aware of the safety and appropriateness of the participation. Participation in activities with the Company, due to the nature of these activities, increases the risk of injury or death for you, your family members and / or other persons, whether in person or online. Most of these activities involve the use of dangerous or dangerous substances such as drugs, alcohol or drugs of any kind. If you can reasonably participate and do so in a comfortable manner, you may choose not to participate in the activities at your discretion and at the discretion of Re LeaSor, if you are willing and able to do so. Participation in activities with this company, whether personal or online, will also inevitably put you and your company in an area that treats, touches or comes into contact with any surface or area of bodily fluids, blood, urine, saliva, semen, feces or other bodily fluids.
